A Case of Strongyloidiasis with Hyperinfection Syndrome

Abstract
A 68-year old Korean male was admitted to Inha hospital with clinical complaints of asthma, vomiting and anorexia. At the first admission, laboratory data revealed eosinophilia and elevation of IgE level.. From the second day of hospitalization,
methylprednisolone and prednisolone treatment was started to cure patient's asthma an continued for 2 months until his third admission due to asthma, vomiting, anorexia, constipation. Oliguria and general weaknes. On the second day of his third
admission, hemorrhagic and inflammatory responses around patient's duodenum were observed through the endoscopic examination. The larval and adult forms of nematodes parasitizing the duodenal cryptic areas with eosinophilic infiltration were
found
in
the tissue sections of biopsied material.
Stool examination revealed rhabditiform and filariform larae of Strongyloides stercoralis in the stool specimens kept at room temperature for a week without fecal cultivation. The patients was diagnosed as hyperinfection syndrome due to S.
stercoralis
infection, and albendazole treatment was started and continued for 7 days. A total of 16,850 parasitic adult females were collected from one-day diarrheal stool after chemotherapy. However, the patient was discharged without complete recovery on
the
46th day of his third hospitalization.
